Comunicare Solutions is a spin-off of the University of Liège (Belgium) aiming to enable better communication between patients and caregivers by enforcing patient education, therapeutic compliance, outcome measurement, and preventing risky situations.

Our patient companioning platform provides patients with tailored health literacy and monitoring tools for self-efficacy so as to engage in a shared decision-making process with their care providers.

COMUNICARE

COMUNICARE is a chronic patient companioning platform grounded in the scientific evidence that the quality of care improves when the patient plays an active role. It provides patients with tailored health literacy and monitoring tools for self-efficacy.

The solution leverages on a mobile application for patient self-management and a follow-up dashboard for caregivers which provides data and analytics based on patient-reported outcomes (e.g. i.e. vital signs, symptoms, and experience).

For its COPD care path, the company has developed a predictive model for the timely detection of exacerbation and hospitalisation risks.
